Employment References (2 required):
References must be completed by a past or current employer, who has the ability to evaluate past skills and performances. Annually, a Competency Assessment/Performance Evaluation is required and must be completed with the same criteria as an Employment Reference. (The Joint Commission, Standard HR.4)
Interview:
A Nurse Manager will perform an interview with applicants to determine employment eligibility. (Joint Commission, Standard HR.2, a)
Backgrounds:
A background is processed prior to employment. It includes dependent adult/child abuse, criminal history and sexual abuse registry and is completed on each applicant. (The Joint Commission, Standard HR.1)
Phase 2: Post Interview
License/Certification:
Licensure is verified with the primary source by a letter or secure electronic verification, according to law and regulations. (Joint Commission, Standard HR.4)
Certifications:
Submit copies (front & back) of all applicable clinical certifications (BCLS, ACLS, PALS & NRP. These certifications should be American Heart Association, Healthcare Provider). (Joint Commission, Standard HR.2, a)
Mandatory Reporter: Child and Dependent Adult Abuse:
All HCPs working in Iowa must hold curriculum approved Mandatory Reporter: Child and Dependent Adult Abuse. (Iowa Code section 235B.16 – Dependent Adult Abuse – Person 18 or older)(Iowa Code section 232.69 (3) – Child Abuse 0 – 18 years old)
Health Examination & TB Screening:
Employees shall have a physical examination within 6 months of employment then every 4 years thereafter or facility specific. Tuberculin test are required at the time of employment and annually thereafter or facility specific. (Inspections and Appeals 58.10 (3))
Immunization Record:
Submit proof or signed Immunization Record of the following: MMR, Varicella and Hep B and Declination. Tetanus is Facility Specific. (CDC, Joint Commission, Standard HR.1, OSHA)
Identification Badge:
A 2 x 2 passport photo is required. A photo identification badge will be created and mailed to each applicant. Healthcare Professionals must identify themselves through the use of identification badges. (Iowa General Assembly: 147.162)
Mandatory Education - GrapeTree University:
Our organization is utilizing and delivering e-learning to staff at various levels. The primary purpose is to provide learning and review of regulatory requirements for The Joint Commission, OSHA and other State, Federal and GrapeTree Medical Staffing requirements. Training is required at the time of application and yearly thereafter. (Joint Commission, Standard HR.1, c, OSHA, State and Federal regulatory agencies)
Employment Eligibility:
An Employment Eligibility Verification (I-9) with Identification Documents must be completed, signed and submitted to the office within 3 days of accepting a position with GrapeTree. The I-9 may also be completed at our office. Please see I-9 form instructions relating to documentation required. (U.S. Department of Justice, Immigration and Naturalization Services)
Electronic Payment Authorization:
Authorization Agreement for Automatic Payroll Deposit with a voided check must be signed and submitted before employment. This process prevents lost checks and allows us to pay frequently. We utilize auto deposit for weekly paychecks.
W-4's:
A Federal and State(s) W-4 Employee’s Withholding Allowance Certificate must be completed and signed before employment. (IRS and Department of Treasury)
